Pop Warner Registration Fields Help Page
Important: The following instructions apply to both football and cheer, although some fields
may only be relevant to one sport. Any additional details or sport-specific questions for football or cheer
will be clearly indicated within the registration.
Player Information (Section 1)
- Most of this information should be filled in automatically, unless you are adding a new child.

If the athlete had a photo on their profile previously, it will load automatically. If this photo is
no longer accurate, please upload a new headshot. (You can crop your photo during the upload process.)

If your athlete played Pop Warner previously and your MARA account successfully linked to the Pop
Warner profile, then the Birth Certificate should show as complete. Otherwise, you will need to
upload a copy of the birth certificate.

Note: Uploading the birth certificate now is not required to complete registration, but it
will be required at some point before the season begins.
Parent Information (Section 2)

Family Role (Parent 1): This is required by Pop Warner—choose the best option from
the dropdown.
- Parent 1 information comes from the MARA account profile.
- Please use the remaining fields to add Parent 2’s information to the player’s Pop Warner record.
Medical Information (Section 3)
- Medical Doctor Signature – Not required for the registration process.
- Player Contract – Not required for the registration process.

Report Card

Since registration is open while school is still in session, full-year report cards are
typically not available until June or after the school year ends.

You must upload your report card before the 2025 season starts to confirm eligibility. A
staff member will then visually verify the uploaded document.

Information on report cards and the Pop Warner All-American Scholar program is found below.
Medical Information Cont (Section 4)
- The Secondary Contact section is for listing the emergency contact.
- Medical Insurance information is required.
- Family Doctor information is required.
- You must answer the required questions about medical conditions, including any history of concussions.
- The athlete’s weight is required to complete the medical information.
Grade/GPA Information (Section 5)

GPA Score – Enter an estimated GPA on a 100% scale. If school year is still in
session... just provide an estimate.

Fall 2025 Grade – Provide the grade level the athlete will be in during the 2025
season. This, along with the athlete’s GPA, determines eligibility for the Pop Warner All-America
Scholar program.

The 2025 Medical History and
2025 Participant Contract and Parental Consent forms require a digital signature.
- Returning players should find the updated forms process much smoother compared to previous years.
Player Information (Section 6)

Is Participant New or Returning – Did they participate in Pop Warner for MARA last season?

Years of Experience – How many years of cheer/tackle football do they have?

Jersey Size / Short Size – Select from the dropdown. (Cheer will have additional size
options such as shoes, skirt, etc.)

Coach Request / Special Requests Comments – Not required.
Pop Warner requires these questions, but requests are not guaranteed.
Even for returning players, most special requests cannot be fulfilled.
Acknowledgement and waiver Information (Section 7)
- This section contains final waivers and acknowledgments.
- The remaining items are standard MARA waivers.
- All these waivers are acknowledged and signed digitally.
